Holiday Honor Card

If you would like to make a $25 or more donation to A.G. Rhodes Health & Rehab in honor of someone special for the holidays, please fill out the fields below by December 15.  For each $25 donation you make, we will send a Holiday Card to the recipient of your choice. 

If you wish to pay by check, please make your check payable to: Foundation for Rhodes Homes, Inc., 2801 Buford Hwy NE, Atlanta, GA 30329, and include the contact information for your card recipients with your check, or send an email to Donations@agrhodes.org with the information. Your gift is tax-deductible to the full extent of the law and you will receive acknowledgement of your gift for your tax records.
